Feedback Welcome on Draft Service Delivery Model

Hospice donors, community residents and health service providers to review

Hospice donors, community residents and other health care service providers are now invited to review a DRAFT Service Delivery Model proposing how hospice services in Northumberland County could be integrated in the “basket of services” currently provided by Community Care Northumberland.

Over the summer, representatives from Hospice Northumberland Lakeshore (HNL), Palliative Care Campbellford (PCC), Community Care Northumberland (CCN) and the Central East LHIN have developed a DRAFT Service Delivery Model that has been shared with staff and volunteers from these organizations and with a team of peers for review and feedback.

This facilitated integration began when Hospice Northumberland Lakeshore and Palliative Care Campbellford partnered with the LHIN to find a way to continue providing these important services when a review of the agencies’ operations revealed the potential to streamline operations and deliver services more effectively. The LHIN asked Community Care Northumberland to be part of the integration because of their local expertise in delivering other community support services to the residents of Northumberland County.

Feedback on the DRAFT Service Delivery Model from hospice donors, community residents and other health care service providers is now being collected via a web-enabled survey and will be reviewed by the respective boards as they approve the model. It will also be accepted in written form sent via mail, email or fax. Feedback must be submitted by August 31, 2010.
